Case Management

Case management services are provided to the clients at THE PROGRAM to help them address issues or concerns that have been neglected, overlooked or not addressed properly.  The primary role of the Case Manager is to work with the clients to identify these issues and prioritize them based upon importance.  It has been found that if these outside concerns are not addressed, they can have a negative impact on the individual’s treatment and recovery.  Case Managers are essential in helping clients become more responsible and better equipped to deal with circumstance that will occur in everyday life.  Some of the areas that Case Managers at THE PROGRAM assist clients in are:

  • Medication Management
  • Assistance with Meetings/Locating meetings
  • Mental Health Referrals
  • Insurance Management
  • Aftercare Referrals and Housing Assistance
  • Preparing for reintegration into society
